ABSTRACT

OBJECTIVE: To investigate, within a private health insurance, the ordering frequency and the costs related to inappropriate tumor markers test orders. METHODS: This study analyzed data regarding tumor markers requests within a private health insurance between 2010 and 2017. Patients included in this analysis were ≥ 50 years old, had available medical records, and had at least 1 tumor markers tested within the study period. Tests were considered inappropriate when tumor markers were used in screening for neoplasms, ie, when there was no previous diagnosis. We evaluated data regarding age, sex, the ordering physician's medical specialty, and test costs. RESULTS: Between 2010 and 2017, 1112 tumor markers tests were performed and increased from 52 to 262 per year. Our sample consisted mostly of women (69.50%) with a mean age of 59.40 (SD 8.20) years. Most orders were inappropriate (87.80%) and represented 79.40% of all expenses with tumor markers tests. Cardiology professionals were the medical specialty that requested the most tumor markers tests (23.90%), followed by internal medicine specialists (22.70%) and gynecologists (19.20%). CONCLUSIONS: We observed a high percentage of inappropriate test orders in the study period, resulting in elevated costs. Studies of this nature deserve the attention of health care managers, and interventions should be performed in order to reduce the inappropriate use of tumor markers tests in clinical practice.


OBJETIVO: investigar no âmbito de um plano de saúde privado a frequência de solicitação e os custos relacionados à solicitação inapropriada de marcadores tumorais. METODOLOGIA: Utilizou-se a base de dados de um plano de saúde privado entre os anos de 2010 a 2017. Foram incluídos na pesquisa, sujeitos com idade ≥ 50 anos, que apresentavam prontuários médicos acessíveis e que havia realizado a dosagem de algum marcadores tumorais no período. Considerou-se como "exame inapropriado" quando o marcador tumoral foi utilizado como rastreio de neoplasia, ou seja, quando não havia o diagnóstico prévio. Foram avaliados os dados referentes à idade, sexo, especialidade do médico solicitante e informações sobre os custos desses exames. RESULTADOS: Foram realizados um total 1.112 testes no período, representando um aumento de 52 para 262 exames/ano. A amostra foi composta na maioria pelo sexo feminino (69,50%), com média de idade de 59,40 ± 8,20 anos. A maioria das solicitações foram inapropriadas (87,80%). Notou-se que a solicitação desses exames, impactaram cerca de 79,40% dos gastos totais do plano de saúde com marcadores tumorais. Os cardiologistas foram a especialidade que mais solicitaram marcadores tumorais em 23,90% das ocasiões, seguidos pelos especialistas em clínica médica (22,70%) e ginecologistas (19,20%). CONCLUSÕES: Observamos um alto percentual de pedidos de exames inadequados no período do estudo, resultando em custos elevados. Estudos dessa natureza merecem a atenção dos gestores de saúde e intervenções devem ser realizadas a fim de reduzir o uso inadequado de testes de marcador tumoral na prática clínica.

ABSTRACT

Abstract Objective To investigate the patterns of hospital births in the state of Rio de Janeiro (RJ), Brazil, between 2015 and 2016; considering the classification of obstetric characteristics proposed by Robson and the prenatal care index proposed by Kotelchuck. Methods Data obtained from the Information System on Live Births of the Informatics Department of the Brazilian Unified Health System (SINASC/DATASUS, in the Portuguese acronym) databases were used to group pregnant women relatively to the Robson classification. A descriptive analysis was performed for each Robson group, considering the variables: maternal age, marital status, schooling, parity, Kotelchuck prenatal adequacy index and gestational age. A logistic model estimated odds ratios (ORs) for cesarean sections (C-sections), considering the aforementioned variables. Results Out of the 456,089 live births in Rio de Janeiro state between 2015 and 2016, 391,961 records were retained, 60.3% of which were C-sections. Most pregnant women (58.6%) were classified in groups 5, 2 or 3. The percentage of C-sections in the Robson groups 1, 2, 3, 4, 5 and 8 was much higher than expected. Prenatal care proved to be inadequate for women who subsequently had a vaginal delivery, had an unfavorable family structure and a lower socioeconomic status (mothers without partners and with lower schooling), compared with those undergoing cesarean delivery. For a sameRobson group, the chance of C-section increases when maternal age rises (OR = 3.33 for 41-45 years old), there is the presence of a partner (OR = 1.81) and prenatal care improves (OR = 3.19 for "adequate plus"). Conclusion There are indications that in the state of RJ, from 2015 to 2016, many cesarean deliveries were performed due to nonclinical factors.


Resumo Objetivo Investigar os padrões dos partos hospitalares no estado do Rio de Janeiro (RJ), Brasil, entre 2015 e 2016, considerando a classificação de características obstétricas de Robson e a dos cuidados pré-natais proposta por Kotelchuck. Métodos Dados sistema de informações sobre nascidos vivos (SINASC) do departamento de informática do sistema único de saúde (DATASUS) foram utilizados para agrupar gestantes relativamente à classificação de Robson. Foi efetuada uma análise descritiva para cada grupo de Robson, considerando-se as variáveis idade materna, estado civil, escolaridade, paridade, o índice de Kotelchuck de adequação do pré-natal e a idade gestacional. Também foi realizado o cálculo de razão de chances (RC) para parto cesáreo, considerando-se um modelo logístico. Resultados Dos 456.089 nascimentos vivos ocorridos no RJ de 2015 a 2016, foram incluídos 391.961 registros, sendo 60,3% cesáreas, com maioria de gestantes (58,6%) nos grupos 5, 2 ou 3. O percentual de cesáreas nos grupos 1, 2, 3, 4, 5 e 8 foi bem superior ao sugerido pela literatura. Para gestantes de um mesmo grupo (controladas as demais características), a chance de cesárea se eleva quando aumenta a idade materna (RC = 3,33 para 41-45 anos), existe a presença de um companheiro (RC = 1,81), o nível de escolaridade é maior (RC = 3,11 para ≥ 12 anos) e o pré-natal é mais cuidadoso (RC= 3,19 para "adequado plus"). Conclusão Há indícios que no RJ, de 2015 a 2016, muitos partos cesáreos foram realizados sob influência de fatores extraclínicos.

Abstract Objective The objective of the present study was to explore obstetric management in relation to clinical, maternal and child health outcomes by using the Robson classification system. Methods Data was collected from obstetrics registries in tertiary care hospitals in Dubai, United Arab Emirates (UAE). Results The analysis of > 5,400 deliveries (60% of all the deliveries in 2016) in major maternity hospitals in Dubai showed that groups 5, 8 and 9 of Robson's classification were the largest contributors to the overall cesarean section (CS) rate and accounted for 30% of the total CS rate. The results indicate that labor was spontaneous in 2,221 (45%) of the women and was augmented or induced in almost 1,634 cases (33%). The birth indication rate was of 64% for normal vaginal delivery, of 24% for emergency CS, and of 9% for elective CS.The rate of vaginal birth after cesarean was 261(6%), the rate of external cephalic version was 28 (0.7%), and the rate of induction was 1,168 (21.4%). The prevalence of the overall Cesarean section was 33%; with majority (53.5%) of it being repeated Cesarean section. Conclusion The CS rate in the United Arab Emirates (UAE) is higher than the global average rate and than the average rate in Asia, which highlights the need for more education of pregnant women and of their physicians in order to promote vaginal birth. A proper planning is needed to reduce the number of CSs in nulliparous women in order to prevent repeated CSs in the future. Monitoring both CS rates and outcomes is essential to ensure that policies, practices, and actions for the optimization of the utilization of CS lead to improved maternal and infant outcomes.

ABSTRACT Purpose: To assess the accuracy of prostate-specific antigen (PSA) adjusted for the transition zone volume (PSATZ) in predicting prostate cancer by comparing the ability of several PSA parameters in predicting prostate cancer in men with intermediate PSA levels of 2.6 - 10.0 ng/mL and its ability to reduce unnecessary biopsies. Materials and Methods: This study included 656 patients referred for prostate biopsy who had a serum PSA of 2.6 - 10.0 ng/mL. Total prostate and transition zone volumes were measured by transrectal ultrasound using the prolate ellipsoid method. The clinical values of PSA, free-to-total (F/T) ratio, PSA density (PSAD) and PSATZ for the detection of prostate cancer were calculated and statistical comparisons between biopsy-positive (cancer) and biopsy-negative (benign) were conducted. Results: Cancer was detected in 172 patients (26.2%). Mean PSA, PSATZ, PSAD and F/T ratio were 7.5 ng/mL, 0.68 ng/mL/cc. 0.25 ng/mL/cc and 0.14 in patients with prostate cancer and 6.29 ng/mL, 0.30 ng/mL/cc, 0.16 ng/mL/cc and 0.22 in patients with benign biopsies, respectively. ROC curves analysis demonstrated that PSATZ had a higher area under curve (0,838) than F/T ratio (0.806) (P<0.001) and PSAD (0.806) (P<0.001). With a cut-off value of 0.22 ng/mL/cc, PSATZ had 100% of sensitivity and could have prevented 24% of unnecessary biopsies. Conclusions: PSATZ may be useful in enhancing the specificity of serum PSA. Compared to other PSA related parameters, it was better in differentiating between prostate cancer and benign prostatic enlargement. Also, PSATZ could reduce a significant number of unnecessary biopsies.

ANTECEDENTES: En las últimas décadas se ha producido un incremento extraordinario en la incidencia de cesáreas en todos los países desarrollados. La tasa de cesáreas en España registrada por el Ministerio de Sanidad es de 24,9%. OBJETIVO: Analizar las cesáreas en el Hospital La Inmaculada, mediante la clasificación de Robson para comparar con otros hospitales, tanto nacional como internacionalmente y establecer las intervenciones posibles para reducir la tasa de cesáreas. MÉTODOS: Se realiza una auditoría retrospectiva de cesáreas en función de la clasificación de diez grupos establecida por Robson desde el 1 enero de 2006 al 31 de diciembre de 2013. RESULTADOS: Se han analizado 9337 partos y 1507 cesáreas con un 16,14%. En la contribución al porcentaje de cesáreas en primer lugar con 25,2% corresponde a nulíparas con un feto único en presentación cefálica, de 37 semanas o más de embarazo. En segundo lugar las multíparas con al menos una cesárea previa, con un feto único en presentación cefálica, de 37 semanas o más de embarazo con 19,4%. En este grupo se ha realizado un 42,2% de cesáreas. En tercer lugar nulíparas con un feto único en presentación cefálica, de 37 semanas o más de embarazo, que han iniciado el parto de forma espontánea con 17,4%. CONCLUSIONES: El aumento de cesáreas en las últimas décadas hace necesario la realización de auditorías mediante un sistema de clasificación, como Robson, para establecer los grupos en los que es posible disminuir el número de cesáreas.


INTRODUCTION: In recent decades there has been a dramatic increase in cesarean section rate in all developed countries. The cesarean section rate in Spain, registered by the Ministry of Health was 24.9%. OBJECTIVE: To analyze cesarean section rate in Inmaculada Local Hospital, using Robson's classification to compare it with other hospitals, both at national and international level and establish potential interventions to reduce such rate. METHODS: A cesarean section rate retrospective audit was performed according to Robson's classification, from 1st January 2006 to 31st December 2013. RESULTS: 9337 deliveries and 1507 cesareans were analyzed on that period of time. The cesarean rate was 16.14%. Nulliparous women with a singleton pregnancy in cephalic presentation at 37 weeks or more and undergo a labor induction before the onset of labor represent the first group of the cesareans with 25.2% of the total. In second place comes multiparous women with at least one previous cesarean section with a singleton pregnancy in cephalic presentation, 37 weeks or more, representing 19.4% of the total cesareans. This group had a 42.2% cesarean rate. In third place (17.4%) we had nulliparous women with a singleton pregnancy in cephalic presentation, 37 weeks or more, who have started labor spontaneously. CONCLUSIONS: Increased cesarean section rate in recent years required audits using a classification system, like Robson's one, to establish the groups in which it is possible to reduce the number of cesareans.

FUNDAMENTO: Buscando aprimorar a utilização dos exames de ecocardiografia, foram criados os Critérios de Adequação (CA) (Appropriateness Criteria), pela Sociedade Norte-Americana de Ecocardiografia (ASE) /Colégio Americano de Cardiologia (ACC). OBJETIVO: Comparar o perfil de adequação de solicitação de ecocardiograma transtorácico (ETT) de acordo com os CA, entre Hospital Público Universitário (HU) e Hospital Privado (HP), e verificar quais características se associam com melhor perfil de solicitação de ETT. MÉTODOS: Foram avaliados prospectivamente 779 solicitações consecutivas de ETT no HP (49,8 por cento) e no HU (50,2 por cento). Sendo 55,6 por cento solicitações para mulheres e 44,4 por cento, para homens com 59,1 ± 15,7 anos. As indicações foram classificadas como: apropriadas, inapropriadas ou não classificadas, e a adequação aos CA foi correlacionada com idade, sexo dos pacientes, além do tempo de formado do solicitante. A análise estatística empregou Coeficiente Kappa e teste Qui-quadrado. RESULTADOS: Não houve diferença significativa no perfil de adequação de solicitações apropriadas de ETT nas duas instituições (71 por cento x 75 por cento; p = 0,3). No HP, os fatores associados a maior taxa de exames apropriados foram: sexo feminino (p = 0,001) e idade menor que 60 anos (p < 0,001). No HU, médicos formados entre 5 e 10 anos apresentaram maior taxa de solicitações inadequadas (p = 0,02). As variáveis que foram preditoras independentes de exames apropriados foram, no HP: sexo feminino (p = 0,001) e idade < 60 anos (p = 0,001). CONCLUSÃO: Nesta avaliação, HP e HU apresentaram perfis de adequação de solicitações similares. Sexo feminino, tempo de formado do médico solicitante e a idade do paciente influíram na adequação das solicitações.


BACKGROUND: Aiming at improving the use of echocardiography tests, the Appropriateness Criteria (AC) were created by the American Society of Echocardiography (ASE)/American College of Cardiology (ACC). OBJECTIVE: To compare the appropriateness profile of transthoracic echocardiography (TTE) requests in accordance with the AC, between a public University Hospital (UH) and a Private Hospital (PH), and verify which characteristics are associated with a better TTE request profile. METHODS: We prospectively assessed 779 consecutive TTE requests in a PH (49.8 percent) and a UH (50.2 percent), with 55.6 percent of requests for women and 44.4 percent for men, aged 59.1 ± 15.7 years. The indications were classified as appropriate, inappropriate or non-classified, and adequacy to AC was correlated with age, patient gender, and the time since graduation of the requesting physician. The statistical analysis used Kappa coefficient and chi-square test. RESULTS: There was no significant difference regarding the adequacy profile of appropriate TTE requests in the two institutions (71 percent vs. 75 percent, p = 0.3). At PH, the factors associated with higher rates of appropriate tests were: female gender (p = 0.001) and age younger than 60 years (p <0.001). In the UH, physicians who had graduated between 5 and 10 years before had a higher rate of inappropriate requests (p = 0.02). The variables that were independent predictors of appropriate tests in the PH were: female sex (p = 0.001) and age <60 years (p = 0.001). CONCLUSION: In this evaluation, the PH and the UH profiles showed similar request appropriateness profiles. Female gender, time since graduation of the requesting physician and the patient's age influenced the appropriateness of requests.

La apendicectomía es la intervención quirúrgica más frecuente en los servicios de urgencia. La clínica, fundamental en el diagnóstico de apendicitis aguda, no siempre es categórica, debido a que los síntomas y signos pueden resultar ambiguos. No obstante, los métodos de laboratorio e imagenológicos no han logrado superarla en cuanto a rendimiento, por lo que sólo cumplen un rol de ayuda en casos específicos. El objetivo del presente estudio es mostrar y analizar los casos de pacientes apendicectomizados con diagnóstico de apendicitis aguda, cuyo informe histopatológico de la pieza operatoria resultó negativo para dicha patología. Se estudian retrospectivamente 677 casos mediante revisión de los informes de biopsias correspondientes al total de los pacientes operados en el Hospital Clínico Regional Valdivia entre enero y diciembre de 2004. Se revisan las fichas clínicas de los casos con diagnóstico histopatológico negativo, analizando luego mediante una planilla Excel variables como edad, sexo, procedencia, clínica, parámetros de laboratorio, período de evolución, tiempo de hospitalización, hallazgos intraoperatorios, diagnóstico histopatológico y morbimortalidad quirúrgica. Se realizaron 106 apendicectomías en blanco (15,7 por ciento), correspondiendo 54 por ciento a mujeres. La edad promedio fue de 23,5 años (1-77). Una paciente presentaba un embarazo de 23 semanas (0,94 por ciento). El tiempo de evolución preoperatorio presentó un promedio y mediana de 1,96 días (1-11). En el 38,7 por ciento de los casos se registró fiebre. A un 73,6 por ciento de los pacientes se les solicitó hemograma preoperatorio. De éstos, un 67.9 por ciento presentó leucocitosis. Un 46,2 por ciento de los pacientes son intervenidos dentro de las primeras 24 horas de evolución. Sólo en un 22,6 por ciento el cirujano concluye en el intraoperatorio una apendicectomía en blanco, evidenciándose en 5 casos (4,7 por ciento) un cuadro diferente a patología apendicular. ABSTRACT

Irrational prescribing is a global phenomenon. The objective of the study was to find out the prescribing practices of dental prescribers in a tertiary care teaching hospital with special emphasis on the utilization of antimicrobial agents. A prospective study was conducted in the month of March 2000. A total of 491 prescriptions were collected randomly. Prescribing pattern was analyzed using WHO basic drug indicators. The average number of drugs for prescription was 2.4. 78.8% of all prescriptions contained antimicrobial agents. It was most commonly prescribed (40.37%) group of drugs followed by anti-inflammatory and analgesics (33.8%). Fixed dose combination of ampicillin and cloxacillin was most commonly prescribed antimicrobial agents. Prophylactic use of AMA (78%) was more than therapeutic purpose (21.9%). Prophylactic use of antimicrobial agents was irrational in all the cases as duration for the use of antimicrobial agents was 5.1 +/- 0.5 days. Fixed dose combinations (45%), drugs by brand name (98.5%) were frequently used. Drug prescribed from Essential Drug List was maximum when one drug was prescribed. Results indicate that there is a scope for improving prescribing habits and minimizing the use of antimicrobial agents. This could be facilitated by periodic education to the prescribers.
